Brand mention
An answer names your brand. Check that it refers to your business, then record the prompt, engine, market and date. A mention does not mean your website was used as a source.

Source citation
An answer links to or explicitly attributes your website. Inspect the cited page and the claim it supports. A citation is evidence of inclusion in that answer, not proof of a visit or inquiry.
Compare the same prompt set and completed answers across periods. Keep failed checks separate and state the denominator behind each rate.
